General Athletic Information: Before every athletic season begins there are Parent/Athlete Meetings that detail general athletic rules and policies. In addition, Head Coaches are introduced, and information regarding program policies, events, schedules, costs and tryouts are explained. This is also a good time to ask any questions to the coach regarding the program. It is highly recommended that parents and athletes attend these meetings for each season. Each meeting is approximately one hour in length. Spring Parent/Athlete Meeting Dates and Times:

- All Athletic Paperwork must be turned in before the season begins. All paperwork should be turned in at the Parent/Athlete Meeting or to the Athletic Office.
- If paperwork is not completed and on file the student-athlete will not be able to practice or tryout.
- Athletic Paperwork is not sent home in 9th grade registration packets you must print it from the link to the right.
Athletic Paperwork:
There are 4 forms that need to be on file each year in order to participate in athletics.
- Physical Examination Form must be on file in the Athletic Office and current within the past three years prior to the start of athletic season.
- Emergency Form must be turned in at the beginning of every season of participation
- Chemical/Drugs/Tobacco and Harassment Agreement Form must be turned once every year prior to the start of the season.
- 2011-12 Minnesota State High School League Eligibility Form and Annual Health Form must be turned in once every year prior to the start of the season. The forms are the last two pages of this document.
Concussion Baseline Registration - Please click on link to read about baseline concussion testing. This mailing went home to all CDH familes July 18, 2011.
Concussion Baseline Testing is a computerized exam call ImPACT (Immediate Post Concussion Assessment and Cognitive Testing). The purpose of the exam is to provide data to physicians if a student-athlete is believed to have suffered a concussion during practice or competition. After a head injury, the exam is taken again and the date is compared to the baseline test.
- Registration must be completed online. Click on testing date below to register: - 20 students will be tested at a time. - Spring student-athletes will be tested. - Free for CDH student-athletes - The testing location will be in the CDH commons.
Co-Curricular Paperwork: All Co-Curricular Paperwork must be on file in order to participate in any activity. Students that have not turned in all the paperwork and have incomplete files will not be able to practice and/or tryout. It is highly recommended that you turn in paperwork at Parent/Athlete Meetings in order to ensure each student-athlete is eligible prior to the start of the season. 2011 -2012 Physical Examination Form It is suggested that physicals examinations occur once a year. However, physicals current within the last three years can be turned in.

2011 - 2012 Athletic Emergency Form Emergency Forms are carried by each coach/facilitator to ensure contact information in the case of an emergency. This form is also kept on file in the training office for athletic purposes.
One emergency form must be turned in each season at the Parent/Athlete Meeting. In the past, only one emergency form was turned in. However, to ensure the coach has this important information, it is required that you turn in one emergency form each season/sport . You may want to make copies of the emergency form to turn in for each season of participation.
This form must be signed by the student and parent/guardian

2011-12 Chemical, Drugs, Tobacco and Harassment Policy The purpose of the Chemical, Drugs, Tobacco, and Harassment Agreement Form is to ensure that student-athletes and parent(s)/guardian(s) are aware of CDH's Policy and the consequences associated with chemical, drugs, tobacco and harassment violations.
This form must be turned in once every year and signed by student and parent/guardian

2011-2012 MN State HS League Eligibility Form and Annual Health Questionnaire The Minnesota State High School League (MSHSL) is the governing body of student-athletes in the State of Minnesota. This form outlines specific rules and policies of the MSHSL to ensure proper eligibility for all student-athletes.
This form has two parts . The first part requires the signatures of parent/guardian and student-athlete indicating a clear understanding of MSHSL eligibility rules as stated in the packet. The second part requires a parent/guardian to verify the health of their daughter/son and give clearance to participate in athletics by completing the Annual Health Form Questionnaire and signing at the bottom along with the student's signature.
This form must be turned in once every year and signed by student and parent/guardian
